ACADEMIC:

• All applicants shall be fully licensed to perform Dental Hygienist.
• Must be licensed by the state of Washington.
• Must have at least 1 year or more years of experience 
• Must be certified in Cardio-Pulmonary Resuscitation
• Must have an active, unrestricted license to practice as an independent practitioner.
• Must provide a copy of the Drug Enforcement Administration (DEA) Certification

Note:

• Open to discuss the pay rate.
• Mileage and relocation assistance provided if required. 

SCHEDULE:

Monday through Friday; normally provide 1 (6) hour sessions once per week. (a session is defined as one hour) excluding Federal Holidays. 

Work hours will normally be between 8:00 am and 5:00 pm, with a 30-minute unpaid lunch break.

All duties shall be performed in accordance with standards and methods generally accepted within the field of Dental, and compliance with all Federal and State policy and regulations.

Must be registered/licensed with the state in which Dental services will be provided; all licensing requirements/certification/accreditation of the contractor’s staff must be maintained during the entire performance period with proof being submitted at the request of the Governed.
